Stop Flu at School Clinic at Mid-Pacific Nov 14

Flu Vaccination Clinic available for students in grades K to 8 ONLY.
Hello Mid-Pacific Community,
I am very happy to report that there will be a Stop Flu at School program this year as the Department of Health was able to secure the funding. The clinic is scheduled for November 14. In the past 7 years we have benefited from this initiative by having the convenience of receiving the shots here on campus free of charge to students K-8. The increase in flu protected people on campus also provided some protection to the unvaccinated population.

Select our school and enter our password flu1070. When you have completely
finished (please double check that you have completed every question), press "print to sign" then sign and date the document. Return the completed form to your child's teacher. It does not get recorded anywhere online so you MUST print the form and bring it to school before the deadline of October 16
. Late forms will not be accepted. There is no make up date if your child misses the clinic.

I strongly urge you to get you and/or your child vaccinated. If you choose not to get the immunization here at school, your doctor's office, Long's Minute Clinics and HMSA sponsored clinics at Safeway are great places to seek the immunization to keep your winter and spring safe from the influenza illness.
