We are delighted that you’ve chosen to apply to Mid-Pacific Institute. There are nuances in the application process at different grade levels. We’ve tried to make them easy to follow for you and your child. At any time you have questions, do not hesitate to contact us.
Priority Application Deadlines
Preschool and Elementary Applications are longer being accepted
for the 2008/09 school year.
Preschool and Kindergarten: Postmarked by Dec. 1, 2007
Grades 1-5: Postmarked by January 15, 2008
Preschool and Kindergarten Age Requirement
Boys must turn five between June 1, 2007 and June 30, 2008
Girls must turn five between September 1, 2007 and September 30, 2008

You may also request an application form by mail. Please see our contact page for information.
- Submit completed application to the Office of Admissions with the $75 Application Fee (non-refundable). See below for mailing address.
- After December 1, 2007, provide your child's current teacher with the Teacher Reference Report.
- Download the Teacher Reference Report forms below. Teacher Reference Reports are not needed for preschool applicants.
Kindergarten - Grade 1 HAIS Teacher Reference Report
Grades 2-12 HAIS Teacher Reference Report 
- Sign the Teacher Reference Report before forwarding it to your teacher.
- Provide the teacher with a stamped envelope addressed to:

- Forms must be received directly from the teacher.
- Applicants to grade 1-5: Furnish a copy of the final report card from the previous academic year by January 31, 2008.
Assessment Sessions for Preschool and Elementary Applicants
Group and/or individual assessments are required for preschool and elementary school applicants. Assessments are scheduled on Saturdays during the months of January through April. Once your child’s appointment has been scheduled by the Office of Admissions, a letter will be sent confirming the appointment. Individual assessments with non- O‘ahu applicants will be arranged individually.
