Applying to Preschool & Elementary School [Grades K - 5]

We are delighted that you’ve chosen to apply to Mid-Pacific Institute. There are nuances in the application process at different grade levels. We’ve tried to make them easy to follow for you and your child. At any time you have questions, do not hesitate to contact us.

How to Apply

There are several ways for you to apply to Mid-Pacific Institute:

1. Apply online. Click here to apply online.
2. Apply through via mail. Download the Download Application for Preschool to 5th grade here.
3. Call 808-973-5005 to request an application packet.

Admissions Checklist

1. Submit completed application to the Office of Admissions with the $75 Application Fee (non-refundable). See below for mailing address.
2. After December 1, 2009, provide your child's current teacher with the Teacher Reference Report.
3. Download the Teacher Reference Report forms below. Teacher Reference Reports are not needed for preschool applicants.
   Kindergarten - Grade 1 HAIS Teacher Reference Report
   Grades 2-12 HAIS Teacher Reference Report
• Sign the Teacher Reference Report before forwarding it to your teacher.
• Provide the teacher with a stamped envelope addressed to:

• Teacher Reference Reports must be received directly from teachers by January 31, 2010.
4. Applicants to grade 1-5: Submit a final report card from the previous academic year by January 31, 2010.

Assessment Sessions for Preschool and Elementary Applicants

Group and/or individual assessments are required for preschool and elementary school applicants. Assessments are scheduled on Saturdays during the months of January through April. Once your child’s appointment has been scheduled by the Office of Admissions, a letter will be sent confirming the appointment. Individual assessments with non- O‘ahu applicants will be arranged individually.
